When to suspect this adverse reaction?

It depends on clinical situation but transfused patients have a great risk of cardiac arrhythmia or cardiac arrest.

Usual causes?

Rapid infusion of large volumes of cold blood can depress the body temperature and result in metabolic complications, such as acidosis, hyperkalaemia, hypocalcaemia, and so on.

This is rarely seen outside the massive transfusion setting.

Investigations

Perform electrolyte analysis and cardiac monitoring.

What to do?

Slow rate of infusion. Use blood warmers where appropriate. Use fresh blood components.
